New Laboratories
Before beginning research in any new laboratory, the Principal
Investigator must complete a Laboratory Annual Self-Inspection
Form (LASIF). LASIFs are reviewed on a monthly basis at
the full meeting of the SRS. Once a laboratory receives approval
for its LASIF, the Principal Investigator will only be
required to submit the two-page Request to Review Biosafety
Project form, describing the specific procedures used in
any new research projects. The LASIF will must be renewed
on an annual basis, and the Science Information Office
will notify the investigator prior to the expiration of
their laboratory's approval. Please submit any Biosafety
applications directly
to the
Science
Information Office.
Established Laboratories
For each new project, you must complete the two-page Request
to Review Biosafety Project form. Projects are reviewed
on
a rolling
basis and the SRS paperwork may be submitted prior to your
R&D
Committee submission. This form is used for the review
of an individual
project.
The annual resubmission
and approval of your laboratory paperwork will renew
your approval for each project conducted in your laboratory.
The Science Information Office
will notify you prior to the annual expiration of your
laboratory's approval. Please submit any Biosafety applications
directly
to the
Science
Information Office.
